Advance Care Planning

Advance Care Planning is a process for directing medical treatment and interventions at a time in the future when you can no longer make your wishes known. If a sudden event leaves you unable to communicate, this gives you the opportunity to share your values and wishes for treatment. This process starts with conversations between you, family, friends, and your healthcare provider. Advance care planning is for everyone at any point in life.
