Once the form is completed, you will be directed to the first video. You will watch a series of four videos. You must view each video in its entirety. If Medical Weight Loss only is chosen, you will not view the videos and your information will be submitted to us directly. We will call to schedule an appointment with you.
Please choose both options if you are interested in surgical and medical weight loss. You will view the videos in their entirety and your information will be submitted for benefit screening.
