Cardiopulmonary Rehabilitation

The cardiopulmonary rehabilitation program is a medically supervised, comprehensive and evidence-based approach proven to provide patients with optimal results. The program is individually tailored consisting of exercise and education through development of a safe, supervised treatment plan with continual monitoring and reassessment to measure outcomes, improve quality of life and transition to a self-directed home program.

Our multi-disciplinary team evaluates and helps treat patients with cardiac and/or pulmonary disease that may impact their ability to fully function.

Your personal physician, who prescribes and refers you to the program, remains your primary physician and receives monthly reports of progress.

Who Qualifies for Rehabilitation?

  • Coronary intervention (balloon/PTCA stent)
  • Congestive heart failure
  • Heart attack
  • Heart bypass surgery

Pulmonary Rehabilitation

Obstructive Lung Disease with the following:

  • Diagnosis of moderate, severe or very severe COPD
  • Past PFT
  • Persistent Asthma, bronchiectasis, cystic fibrosis, bronchitis and emphysema

OR

Nonobstructive Lung Disease such as any of the following

  • Pulmonary fibrosis
  • Idiopathic pulmonary fibrosis
  • Sarcoidosis
  • Kyphoscoliosis
  • Ankylosing spondylitis
  • ALS
  • Pulmonary hypertension
  • Cystic fibrosis
  • Interstitial lung disease
  • Restrictive lung disease
  • Post COVID

OTHER

Exercise

The primary goal of rehabilitation is to help you attain the exercise level needed for cardiovascular and pulmonary fitness to return to a healthy lifestyle. The optimum level of conditioning can be reached through aerobic exercises. An initial exercise evaluation will be conducted by our staff and tailored to the patient needs.

  • Walking on treadmill
  • Recumbent bicycle
  • Recumbent stepper or elliptical
  • Upper body exercise and/or strengthening as appropriate

Family member are welcome to attend educational classes at no additional charge

  • Coronary artery disease and heart function

  • Eating out/recipe modifications

  • Psychological support for patients and caregivers

Cost

Insurance may cover the costs; if you have questions concerning your coverage, please contact your carrier. You are charged only for sessions attended, INTEGRIS Health will file your insurance claims.

Phases of Cardiopulmonary Rehab

Phase I: Inpatient Program

Phase 1 takes place in the hospital after you have experienced a heart attack or other major heart problem.

  • An assessment of how well you do self-care (such as brushing teeth or hair or bathing).
  • A risk assessment, to determine your ability to participate in an exercise program.
  • Identifying which daily activities, such as lifting, you can do safely.
  • Patient and family education about lifestyle changes you need to make such as eating healthful foods and stopping cigarette smoking.
  • Performing light exercise such as walking short distances several times a day and possibly a weight-training program.

Phase II, III, and IV: Outpatient Programs

  • These remaining three phases of cardiopulmonary rehab take place at our outpatient locations.
  • These facilities, coupled with our team of trained professionals, will help you meet your health goals - a healthy lifestyle including eating a balanced diet, exercising regularly and not smoking–can become a way of life.
  • Outpatient cardiopulmonary rehabilitation is a supervised exercise and education program with progressive conditioning, heart rhythm and rate monitoring and telemetry.
  • These phases will help you transition to a self-directed home program.
  • You will be given the opportunity to improve exercise tolerance and strengthening in a safe, supervised environment while you achieve optimal functional activity level.
